Notice of Privacy Practices
Draft for review. This text has not yet been approved for publication.
This notice applies to Jeffrey Oppenheimer MD P.C., doing business as Kosher Skinny Shotsand as RejuScience (“we”). Effective the date this notice is first posted.
Your rights
When it comes to your health information, you have certain rights. You can:
- Get a copy of your medical record. Ask us for an electronic or paper copy. We will provide it, usually within 30 days, and may charge a reasonable, cost-based fee.
- Ask us to correct your medical record if you think it is incorrect or incomplete. We may say no, but we will tell you why in writing within 60 days.
- Ask for confidential communications: to be contacted in a specific way (for example, a different phone or email) or sent mail to a different address. We will say yes to all reasonable requests.
- Ask us to limit what we use or share. We are not required to agree, except that if you pay for a service in full out of pocket, you can ask us not to share that information with your health insurer, and we will say yes unless a law requires us to share it.
- Get a list of those with whom we have shared your information in the six years before you ask, who we shared it with and why, except for treatment, payment, health care operations and certain other disclosures. One list a year is free.
- Get a copy of this notice on paper at any time, even if you agreed to receive it electronically.
- Choose someone to act for you, such as a medical power of attorney or legal guardian. We will check that the person has this authority before we act.
- File a complaint if you feel your rights have been violated (see below).
Your choices
You can tell us whether to share information with family, close friends or others involved in your care. If you cannot tell us, for example if you are unconscious, we may share it if we believe it is in your best interest, or to lessen a serious and imminent threat to health or safety.
We never share your information for marketing purposes, never sell it, and do not contact you for fundraising, unless you give us written permission. Today we do none of these.
Our uses and disclosures
To treat you
Our physician and the staff who work under his direction use your information to decide whether a prescription is right for you and to care for you. We send your prescription and shipping details to the licensed pharmacy that prepares and ships your medicine.
To be paid
We share what is needed to place a hold on and charge your card with the company that processes card payments for us.
To run our practice
We use your information to run the practice, improve care and contact you about your treatment by email and phone. Companies that do work for us, such as Google, which stores our records, and the services that deliver our email, may receive information only under a written agreement (a business associate agreement) that requires them to protect it.
Other ways the law allows or requires
We may share your information to help with public health and safety issues (such as reporting adverse reactions to medicines), to comply with the law, to respond to lawsuits, legal actions and lawful requests from law enforcement, for workers' compensation and government requests, and with health oversight agencies, in each case only as the law allows.
Our responsibilities
- We are required by law to maintain the privacy and security of your protected health information.
- We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
- We must follow the duties and privacy practices described in this notice and give you a copy of it.
- We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time by letting us know in writing.
Changes to this notice
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be on this page, and we will give you a copy on request.
